Services Offered
To qualify for service you must:
* Be 19 years of age or older.
* Be restricted in some way from performing
activities of daily living.
* At risk of entering a nursing home prematurely.

The Community Based In-Home Services Program was designed to help individuals remain in the community, to foster self sufficiency
and prevent
inappropriate nursing home placement. This program provides services on a part time basis to enable individuals
to remain in their own home, or the home of a family member. All services are provided on a sliding fee scale based on income.

Caregiver Support!
Community Based in Home Services is pleased to offer a Family Care Giver Support group the third Thurdsay
of every month. The group will meet from 6:00 to 7:30 at Central Wyoming Senior Services located at 1831 E. 4th Street.
The group co-facilitated by Social Worker Lily Neiberger offers emotional support and resources for those caring for an ill loved
one.
